Job Summary
The Referral Coordinator LPN at Duke Home Care serves as a key liaison between hospitals, physician offices, and Duke Home Care to ensure the timely and accurate coordination of home health referrals. This hybrid role requires strong clerical and administrative skills, with a heavy emphasis on electronic medical record (EMR) management, chart review, and order entry.
MAJOR JOB RESPONSIBILITIES
Reviews all referrals for Home Health for eligibility for home health, discipline orders and safety concerns.
Manage and Coordinate the Start of Care schedule for Home Health to ensure timely admission to home health and patient safety
Obtains updated orders as needed from providers for referrals
Provide nursing care for patients in accordance with physician?s order and established policies and procedures
Works with home health managers and schedulers to ensure admissions are scheduled timely based on patient need.
Communication with field clinicians regarding scheduled admissions.
Participate in daily team meetings and conferences, as necessary, to facilitate that appropriate services are provided to the patient
Maintain up-to-date knowledge of Home Care Regulations, Insurance and Joint Commission through in-service and workshop attendance
Coordinate care plan activities with multidisciplinary team members as directed by Home Health Assistant Director
Document care and services provided in accordance with DHCH policies, procedures and regulatory standards
Provide documentation that is timely, concise and complete
Communicate effectively with team members, caregivers and physicians
Implement agency policy, procedures and objectives
Maintain confidentiality of employees and patient information
Exhibit an attitude which promotes harmony and goodwill in the workplace
Display a clean, neat and professional appearance
Participate in Performance Improvement Program
Promotes quality, comprehensive services through a team approach
Provide patient care as needed as outlined in the DHCH LPN role
Perform other duties as assigned
Knowledge, Skills, Education, and Experience Required:
Work requires graduation from an accredited Practical Nurse program.
Highly preferred experience in Home Health Setting for 2-4 years.
Degrees, Licensure, and/or Certification
Current State of North Carolina LPN licensure is required or Multi-State Compact License (If out of state compact license, must obtain permanent NC license within 60-days of employment).
Active and current driver's license
BLS Certification is required (If expired, this training will be provided during orientation.)
Working knowledge of Medicare and Medicaid regulations.
Strong computer skills preferred
